On this page, you will find information on all the documents you need to print, complete, sign, and bring with you on your first day at UMUC. Please note: All documents MUST be submitted in black ink, without any errors (i.e., scratches, dashes, correction fluid, etc.).
- Accounting of Service Time
If you are coming directly from or have previously been employed at another state agency, Maryland school of higher education, or USM institution, and contributed to a state pension retirement plan, please complete this Accounting of Service Time form. You must bring this completed form with you on your first day of employment.
- Employment Eligibility Verification and Authorization Form (I-9), including specified identification
The purpose of this form is to document that each new hire (both citizen and noncitizen) hired after November 6, 1986, is authorized to work in the United States. Please review the instructions, then print, complete Section 1 only, and sign the Employment Eligibility Verification and Authorization Form (I-9) (Please note: This document is not viewable from within your browser. You will need to download the PDF document and open it from your computer). Please note that you are required to bring acceptable original documentation with you on the first day.
- Tax and Residency Worksheets
Please complete the Federal Personal Allowance Worksheet and, if applicable, the State of Maryland Withholding Allowance Sheet. An HR specialist will help you complete the appropriate W-4 form on your first day.
- Direct Deposit Authorization Form
To activate direct deposit, you must complete an authorization form. Fill the form out completely. Enter your account number as it appears on your personal check. When you have completed the authorization form, make a copy for your records and bring your completed form to orientation.

- University of Maryland University College Confidentiality of Institutional Data
By signing this form, you acknowledge that you understand the obligation as an employee to maintain the confidentiality of any and all university data, including but not limited to the institutional information and that you understand the associated penalties. This signed confidentiality statement will be maintained in your personnel file.

- MD Substance Abuse Policy and Acknowledgement of Receipt
The Maryland Substance Abuse Policy applies to all state employees. By receipt of this policy, you acknowledge that the unlawful manufacture; distribution, dispensation, possession or use of a controlled substance is prohibited on state owned or utilized premises and violation of this policy can subject you to discipline up to and including termination. As a condition of your employment, you must abide by the terms of this policy and will notify your supervisor of any criminal drug conviction no later than five days after such conviction. You further realize that federal law mandates that the employer communicate this conviction to the federal agency, and you hereby waive any and all claims that may arise from conveying this information to the federal agency.

- UMUC Policy of Acceptable Use and Acknowledgement of Receipt
By signing the attached document, you acknowledge that you have received and read the UMUC Policy on Acceptable Use Policy (270.00). In addition, you understand that at the end of each work day, you will not shut down your computer, but log off only unless directed otherwise by the Office of Information Technology or designee.

- E-Mail Guidelines and Acknowledgement of Receipt
These guidelines provide parameters for the appropriate use of e-mail at UMUC, including information on when you should (and should not) use electronic mail and the privacy issues that exist when you choose to use e-mail. These guidelines ensure that common courtesy and appropriateness are considered when sending e-mail.

- Additional Employment Questionnaire
Please complete the Additional Employment Questionnaire and bring your completed questionnaire to orientation.
- Mandatory Discrimination and Sexual Violence Prevention Training
All new employees are required to complete the updated Sexual Harassment and Sexual Violence Prevention Training modules. A personalized link with information to complete the appropriate training for your job function will be e-mailed to you upon completion of the new hire process. You must complete the Sexual Harassment and Sexual Violence Prevention Training within the first 45 days of employment with UMUC to ensure compliance with UMUC EEO Policies. Once you have completed the training, the program will notify us and you will be able to print your completion certificate.
