You are also responsible for giving your worksite supervisor the student work evaluation form (.pdf) during the twelfth week of your Co-op. Your supervisor will complete the evaluation and return it to your faculty sponsor, who will take this evaluation into consideration when grading your work for the 15-week period. See the student instructions for evaluation (.pdf) for more information.

The last thing you will need to complete is the course and faculty evaluation form (.pdf) Please send the completed form to the Center for Teaching and Learning by the end of week 14.
